Five categories. Everything else is noise.

The AI risk landscape is overwhelming if you try to track every possible failure mode. As an executive, you don't need to be an expert in all of them. You need to understand five categories well enough to ask the right questions and make informed decisions.

1. Accuracy & Reliability

AI systems generate plausible-sounding outputs that are wrong. In customer-facing applications, this means misinformation. In financial analysis, it means bad decisions. In legal contexts, it means liability. The question to ask: "What's our error rate, who catches mistakes, and what's the cost of a wrong answer?"

2. Data Privacy & Security

Every AI system processes data. Where does that data go? Who can see it? Is it used to train third-party models? In regulated industries (healthcare, finance, legal), data handling isn't just a risk, it's a compliance requirement with real penalties. The question: "Exactly what data flows through this system and where does it end up?"

3. Bias & Fairness

AI models reflect the biases in their training data. In hiring, lending, insurance, and customer segmentation, biased AI creates legal exposure and reputational damage. This isn't theoretical; companies have already faced lawsuits and regulatory action. The question: "How do we test for bias in this system, and how often?"

4. Regulatory & Legal

The EU AI Act is in force. State-level AI legislation is multiplying in the US. Industry-specific regulations are evolving rapidly. The regulatory landscape will be substantially different 12 months from now than it is today. The question: "Who on our team is tracking AI regulation, and are we building compliance into design, not bolting it on after?"

5. Intellectual Property

Who owns AI-generated content? Can you copyright it? What if the AI reproduces copyrighted training data in its output? These questions are being litigated right now, and the answers are still forming. The question: "What's our legal position on IP for AI-generated work, and does our legal team have a current opinion on this?"

The executive responsibility: You don't need to solve these risks. You need to ensure someone in your organization owns each one, reports on it regularly, and has the authority to slow down or stop an AI initiative if the risk becomes unacceptable. That's governance.

Building governance that enables, not blocks.

The worst AI governance frameworks are the ones that create so much process that nobody uses AI at all. The second worst are the ones that don't exist, leading to uncontrolled shadow AI usage across the organization. The sweet spot is a lightweight framework that sets boundaries without strangling innovation.

Tier 1: Open Use (Low Risk)
Examples: Internal brainstorming, meeting summarization, first-draft writing, research assistance. Rule: Use approved tools freely. Don't input customer PII, financial data, or proprietary IP. No approval needed. Review frequency: Quarterly audit of tool usage.
Tier 2: Guided Use (Medium Risk)
Examples: Customer-facing content generation, internal data analysis, automated email responses. Rule: Human review required before any output reaches customers or influences decisions. Approved tools only. Review frequency: Monthly spot-checks.
Tier 3: Controlled Use (High Risk)
Examples: Hiring/HR decisions, financial modeling, regulatory compliance, customer data processing. Rule: Requires AI governance review before deployment. Ongoing bias testing. Human-in-the-loop for every consequential decision. Review frequency: Continuous monitoring.

You can implement this framework in one week: (1) List every AI tool currently in use across your organization. (2) Assign each use case to a tier. (3) Write a one-page policy for each tier.
